Cohort-Level Data Instructions & Submission
Applicants must provide cohort level data and comparative data representing achievement scores versus the applicant’s host district and state. Applicants must provide the number of students in each proficiency threshold (i.e. Proficient) across all subjects for each grade and for all years in which State Test data is available. In addition, the number of students in each proficiency threshold for the host district and state must be provided for all the subjects, grades, and years provided for the applicant.
Applicants must also provide basic demographic information for each grade-level entry (i.e. English Language Learners, Free and Reduced Lunch, and Minority). If comparison data is not available for a given grade, subject area, or proficiency level, applicants should leave the space reserved for that data blank.
Cohort-Level Data Template
When submitting cohort-level data, an applicant should upload one Cohort-Level Data Template. If an applicant operates more than one school, the Cohort-Level Data Template allows for different schools or the creation of additional “Achievement Data” tabs.
